Job Description
At Healthcare in Action, our mission is to bring healthcare directly to individuals experiencing homelessness through a mobile, community-based “street medicine” model. Partnering with Medicare and Medicaid health plans as well as hospitals and healthcare systems, we deliver care where it is needed most.
Our approach uses fully equipped medical vans to provide essential healthcare services in locations such as encampments, shelters, parks, and other community spaces where patients reside.
The Clinical Support Partner (Medical Assistant or EMT) plays a key role in supporting clinical teams by assisting patients who are unhoused or at risk. This includes providing hands-on clinical support such as taking vital signs, assisting with blood collection, and performing basic wound care.
In addition to clinical duties, this role involves coordinating patient care, which may include arranging transportation services, managing prescription pickups, and scheduling follow-up appointments. You will also assist with inventory management, supply ordering, and handling incoming patient communications and referrals.
This position is based in the LA Riverbed area.
Key Responsibilities
- Perform patient assessments including vital signs, EKGs, vaccinations, and lab specimen collection when required
- Provide basic wound care and ongoing patient monitoring
- Support the development and implementation of individualized care plans
- Collaborate with patients and care teams including physicians, therapists, and care coordinators
- Educate patients on health maintenance, chronic condition management, and preventive care
- Assist in coordinating referrals to internal and external healthcare resources
- Maintain a safe and supportive care environment aligned with clinical standards
- Ensure proper handling of lab samples, medications, and biohazard materials
- Accurately document all patient interactions in a timely manner
- Participate in quality improvement, audits, and care coordination initiatives
- Monitor and support patients in achieving long-term health goals
- Maintain stock, supplies, and equipment in mobile care units
- Work closely with multidisciplinary teams to improve patient outcomes
Qualifications
- High school diploma or GED required
- Medical Assistant certification or valid EMT license (California) required
- Minimum 2 years of experience in a clinical, hospital, or field healthcare setting
- Ability to perform venipuncture (blood draws)
- Experience with electronic medical records (EPIC preferred)
- Strong clinical assessment and problem-solving skills
- Ability to manage multiple priorities in fast-paced environments
- Excellent communication and interpersonal skills
- Strong understanding of patient confidentiality and HIPAA compliance
- Ability to work independently and within a team setting
- Valid driver’s license and reliable transportation required
- Clean driving record and required insurance coverage
